Required Documents for Enrollment:

1. Immunizations
2. Birth Certificate
3. Proof of Residency showing you live within MHS boundaries.
   1. Examples: PG&E, Water & Garbage or Cable bill.
4. Withdraw grades from previous school and a current transcript
